The Biological Basis of Sex Differences in Athletic Performance: Consensus Statement for the American College of Sports Medicine

Biological sex is a primary determinant of athletic performance because of fundamental sex differences in anatomy and physiology dictated by sex chromosomes and sex hormones. Adult men are typically stronger, more powerful, and faster than women of similar age and training status. Thus, for athletic events and sports relying on endurance, muscle strength, speed, and power, males typically outperform females by 10%–30% depending on the requirements of the event. These sex differences in performance emerge with the onset of puberty and coincide with the increase in endogenous sex steroid hormones, in particular testosterone in males, which increases 30-fold by adulthood, but remains low in females. The primary goal of this consensus statement is to provide the latest scientific knowledge and mechanisms for the sex differences in athletic performance.

Several longitudinal studies in transgender women show a reduction in hemoglobin/hematocrit (the latter equivalent to cisgender women after ~4 months), and lean body mass, muscle cross-sectional area, and muscle strength, but are typically stronger than cisgender women (387) even 14 yr after initiation of GAHT (388). For example, nonathletic transgender women who underwent estrogen treatment for 12 months decreased muscle volume by 5% but maintained muscle strength (384).

Individuals with 46,XY DSD who are born with female or ambiguous genitalia and functioning testes, such as in the case of 5-alpha reductase deficiency, will produce testosterone concentrations in the typical male range (9,325) (Fig. 2). Except in cases of androgen resistance (CAIS), the individual will respond to testosterone made during puberty and adult life with increased virilization and muscle development (9,325). The prevalence of 46,XY DSD is estimated to be ~140 times greater (7.1/1000 vs 0.05/1000) among elite women athletes than the general population (18,404). These findings, together with an increased occurrence of mild forms of hyperandrogenism (PCOS) among elite female athletes (393), support an advantageous role of endogenous hyperandrogenism for athletic performance in women.

International Olympic Committee Was Warned About Male Boxers, World Boxing Organization Vice President Says – Reduxx

A Hungarian sports official has come out and stated that Algerian boxer Imane Khelif is not female. István Kovács, the European Vice President of the World Boxing Organization and former Secretary General of the International Boxing Association, told Hungarian press that he had warned the International Olympic Committee about males participating in women’s boxing as early as 2022, but that nothing was done.

In a shocking statement made to Magyar Nemzet yesterday, Kovács confirmed the speculation surrounding the Algerian boxer, adding that it had been known as early as 2022 that Khelif was biologically male.

“The problem was not with the level of Khelif’s testosterone, because that can be adjusted nowadays, but with the result of the gender test, which clearly revealed that the Algerian boxer is biologically male,” Kovács said in an interview with Magyar Nemzet, adding that a total of five boxers had been examined including Khelif by the International Boxing Association, and all of them “were indeed men.”

Kovács asserted that he personally reported shocking result immediately to the International Olympics Committee, “but as unbelievable as it is, they have not responded to this to this day.” The retired world champion boxer also commented that he recently spoke with former women’s world champion Mária Kovács, who bitterly remarked that in modern women’s boxing, “there is a 20 percent chance that one of the athletes will suffer a testicular injury.”

Male athletes with DSDs are sometimes actively sought out by national coaches because of their tremendous “natural” advantage over biological females.

At the 2016 Rio Olympics, male runners with DSDs won all three top spots in the Women’s 800m race.

World Boxing boss Boris Van Der Vorst addresses gender issues as Angela Carini quits mid-fight against Imane Khelif – ABC News

Khelif won her(sic) first Paris bout on Thursday when her(sic) Italian opponent, Angela Carini, emotionally quit after just 46 seconds and a few punches.

Even though Carini said she wasn’t making a political statement about Khelif, Carini’s tearful abandonment of the bout became a worldwide sensation on social media and in Western culture wars.

Criticism of the two boxers is based partly in the policies and decisions of the IBA, which has been out of the Olympic movement since 2019 after years of IOC concerns about its leadership, integrity and financial transparency.

The IBA disqualified Khelif from its world championships because of what it said were elevated levels of testosterone, and it stripped Lin of a bronze medal because it claimed she(sic) failed to meet unspecified eligibility requirements in a biochemical test.

Rival hits out at ‘incredibly dangerous’ boxers who failed gender tests outside Olympics |The Telegraph

Olympic approval of two boxers who failed gender and testosterone eligibility tests is “‘incredibly dangerous”, according to one leading woman fighter.
Australia’s boxing captain Caitlin Parker voiced her fears as one of her team-mates is fighting in the same 66kg weight category as Imane Khelif.
“I don’t agree with them being allowed to compete in sport, especially combat sports,” Parker said, having just reached the quarter-finals in the 75kg division with a unanimous points victory over Mexico’s Vanessa Ortiz.
“It can be incredibly dangerous. I don’t agree with it. It’s not like I haven’t sparred men before. But you know it can be dangerous for combat sports and it should be seriously looked into.”
Studies show higher testosterone carries a huge safety risk for women in any combat sport.
Men, the science shows, punch 2.6 times harder than women; the biggest documented performance advantage in any sport.
“Yes, biologically … genetically they are going to have more advantages and in combat sports it can be dangerous,” said Parker. “I really hope the organisations get their act together so that boxing can continue to be at the Olympics.
“It’s the oldest Olympic sport. Women’s boxing was only introduced in 2012 and I want to see it for the next 100, 200 years to come.”
Australian boxing coach Santiago Nieva said, in contrast with Parker, that there would be no complaints if Williamson met the Algerian in the ring.

Lib Dems admit discriminating against gender-critical candidate – The Free Speech Union

The Liberal Democrats have admitted discriminating against a mother who was barred from standing as an MP after wearing a T-shirt reading: “Woman: Adult, Human, Female”, reports The Telegraph.

Natalie Bird says she was suspended from the party after being “harrassed” and “smeared” by activists because she fought for women’s rights and expressed her opinion that it is not possible for a person to change sex.

After a three-and-a-half-year legal battle, the party admitted Ms Bird’s claims at the 11th hour before the trial was due to begin next month.

Ms Bird, a domestic abuse survivor who has fought for women-only spaces such as refuges, was first targeted by trans activists in Nov 2017 after she was elected co-chair of the party’s Radical Association.

According to the legal claim, Ms Bird “was bullied by other members of the executive of the Radical Association for expressing her views regarding biological sex and safe spaces for vulnerable women. She was told that she was an ‘illiberal Terf’.” (Terf stands for Trans Exclusionary Radical Feminist and is often used as a slur against those with gender-critical views).

Two athletes competing at the Paris Olympics as “women” were previously disqualified from a women’s world championship for having “XY chromosomes.” Imane Khelif of Algeria and Lin Yu-Ting of Taiwan are scheduled to compete in Olympic women’s boxing next week despite past questions surrounding their biological sex.

While neither have stated they identify as transgender, it is suspected that both are impacted by a Difference of Sexual Development (DSD), a category of medical conditions encompassing any problem noted at birth where the genitalia are atypical in relation to the chromosomes or gonads.

Speaking to Reduxx, a representative with the Independent Council on Women’s Sports (ICONS) condemned the confusion that had emerged surrounding the sex of competitors due to the International Olympic Committee’s 2000 decision to end sex-verification screening.

In the FAQ for the Paris 2024 Boxing Unit, no gender eligibility guidelines are specified, something Smith suggests likely indicates that individual nations were given a tremendous amount of power to deem their own athletes eligible.

“In boxing, the recent contentious split between the IBA and the IOC has now placed Olympic eligibility power into the hands of national boxing federations, allowing countries like Algeria and Taiwan to set their own standards and continue placing male boxers in the ring with female athletes in combat for women’s Olympic medals,” Smith explains.

“The physical abuse of women on an Olympic stage eliminates the integrity of all Olympic events and risks lifelong injury or even death for female athletes. This deceit cannot be allowed to continue.”

Discovering My Gender-Bendy Prenatal History | Reality’s Last Stand

Various hormone-disrupting drugs, used in abundance in millions of pregnancies over many decades, may be quietly distorting human development.

[M]y mother had been prescribed a regimen of synthetic sex hormones during her pregnancy with me because, at the time, it was (falsely) believed that these drugs could prevent miscarriage or mitigate other pregnancy risks. But what specific sex hormones was my mother given? And to what effect?

I was acutely aware of the catastrophe of the fake estrogen drug, diethylstilbestrol (DES), which was administered to more than 10 million pregnant women worldwide from the baby boom era through 1971, and even later in many cases. This lab-made pseudo-hormone had been widely hyped as an anti-miscarriage treatment or a sort of vitamin that helped produced larger, healthier babies. It was even more heavily used in the livestock industry, causing additional low-level human exposure until it was phased out in 1979.

However, instead of producing healthy babies, DES was a teratogenic chemical mutilator later found to increase risk for a laundry list of horrors for the exposed offspring. In females, these included but were hardly limited to, vaginal and cervical cancer, breast cancer, malformed uterus and fallopian tubes, infertility, and miscarriage. In males, it was linked to testicular cancers, penile and testicular abnormalities, and gender identity differences.[1]

I noticed that those of us exposed to progestins (synthetic molecules with progesterone-like effects) and smaller amounts of synthetic estrogens, including DES, had exhibited shifts in personality. We were often described as more independent, sensitive, self-assured, individualistic, and self-sufficient, yet less cortically alert. Those exposed to higher doses of estrogens were more group-oriented, and less independent or self-assured.

The report also detailed the earlier research finding that synthetic sex hormone drugs could masculinize or defeminize females, or demasculinize or feminize males, both in terms of body and behavior.

Exposure to these drugs were not spread evenly throughout the population. A large U.S. study, the Collaborative Perinatal Project, which involving tens of thousands of pregnancies at public hospitals, found that 5 percent of pregnancies in the late 1950s and early to mid-1960s received such treatments. However, rates were much higher in private clinics and hospitals, like the clinic where my mother was treated, where wealthier women could afford these drugs.

These hormone imposters interfered with the natural signaling involved in developing typical male and female brains. In the case of my high-dose progestin group, she noted that we (females) developed “a more masculine configuration.”

Defects in hormonal signaling—whether innate or externally imposed—can lead to deviations from normal sexual differentiation of the brain, body, or both.

Case in point 1, faulty androgen receptors:Androgen insensitivity syndrome (AIS) is a rare genetic disorder in which an XY male with testosterone-releasing gonads (testes) develops along a female or even hyper-feminine pattern.

Case in point 2, virilized girls: The earlier generation of synthetic progestins, used before those given to my mother, had stronger androgenic effects, often causing physical virilization in exposed girls. Severe birth defects included micro-penis-like enlarged clitorises, scrotum-like fused labia, and a conjoined urethra and vagina, known as a urogenital sinus. Unlike natural progesterone, synthetic progestins can interact with androgen receptors, triggering a masculinizing developmental cascade in females.[6]

Case in point 3, estrogenized boys: If a male embryo/fetus experiences low or no exogenous estrogen exposure during the first several months, he is likely to develop a normal male reproductive system. However, if he is whomped with a potent estrogenic drug like DES during mid to late gestation—a critical period of brain development—his brain may develop along a more female-typical pattern. While it’s impossible to have a brain of the opposite sex to your body, the effects of DES can produce cross-sex brain patterns.[7]

[E]ven without a toxic exposure like DES, natural endogenous processes can lead to a non gender-conforming orientation. For instance, research suggests that some women who have successive male children may produce an antibody that increases the likelihood that males later in the birth order will be gay.

Though the use of synthetic estrogens waned in the 1970s, progestin-based protocols remained in widespread use for decades until just last year. In 2023, the FDA finally revoked approval for the last of these drugs, marketed as Makena (technically 17-OHPC, the same progestin in Deluteval and Delalutin). This decision followed clinical trials showing it was ineffective. In May 2024, the European Medicines Agency recommended suspending the marketing authorizations for medicines containing 17-OHPC.

Despite the widespread exposure of individuals to synthetic sex hormones and other hormone-disrupting drugs, knowledge and awareness of their potential fetal impacts has yet to seep into public consciousness.

Recent decades have seen a dramatic 20-fold increase in LGBTQIA+ identities (Figure 3).[10] It seems that narratives about this phenomenon are divided into two camps. One side argues that the escalating rates of gender non-conformity simply reflect greater acceptance of always-existing but previously repressed sexual identities and forms of expression. The other side is concerned that social constructs and contagions might be unduly persuading young people to adopt new gender identities and undergo medical procedures that they may later regret and can cause lasting harm.

However, this debate strikes me as a bit naive, a bit too ideological, and a bit too lacking in context. Neither side seems to contemplate how this surge might in part be the outcome of a long history of drugs subtly reshaping, or at least re-“flavoring,” segments of humanity. The debate should not be limited to “it’s Pride” or “it’s TikTok.” There is also toxicology to consider.

A distraught father begged in vain for a leading plastic surgeon not to remove his 20-year-old daughter’s breasts in a transgender operation, telling him in an excoriating letter that he was about to destroy her life.

The father told consultant Mohsen El Gammal that his daughter suffered depression, and as a way through ongoing mental health problems had previously turned to self-harm and then starving herself before embracing the ‘trans cult’.

In the heartbreaking letter, he tells Mr El Gammal, who boasts of carrying out 1,000 such operations: ‘Ultimately, my daughter is a depressive.’

The father’s release of his letter to Mr El Gammal follows US tycoon Elon Musk revealing he has ‘lost’ his son Xavier, 20, who is now known as a woman called Vivian after being prescribed gender-change drugs in the pandemic.

SpaceX and Tesla owner Mr Musk denounced ‘gender-affirming care’ as ‘evil’.

The father who wrote to Mr El Gammal said: ‘Women and men can be whatever they want to be.

‘They don’t have to conform to anyone’s stereotypes, and by making them do that we’re leading young people to take life-changing drugs and have life-changing surgery at an age when they don’t really get what the long-term consequences are.’

ALLISON BAILEY LOSES APPEAL AGAINST STONEWALL IN SHOCK JUDGMENT

1. Allison Bailey, the barrister who successfully sued her chambers, Garden Court, for direct discrimination when the chambers put her under investigation following a complaint from LGBTQ+ charity Stonewall about the barrister, who is black and a lesbian.

2. Bailey, who does not believe men can become women, came to prominence in October 2019, when she helped to set up the charity LGB Alliance, and after bringing her landmark discrimination case.

3. Bailey was represented by top silk Ben Cooper KC, who argued, in both the Employment Tribunal and the Employment Appeal Tribunal, that Stonewall acted unlawfully when they caused or induced the direct discrimination she suffered at the hands of Garden Court Chambers.

4. The Hon. Mr Justice Bourne, sitting in the Employment Appeal Tribunal, did not agree and dismissed Bailey’s case against Stonewall in a judgment that will shock legal commentators, many of whom believe that Stonewall was lucky to escape legal liability in the first place.

9. In a dramatic twist, Bailey believes that although she has lost her Appeal, the judgment creates legal liability for Stonewall in the area of indirect discrimination and thus further represents a hollow victory for them. Bailey wrote on X/Twitter:
“My case is about direct discrimination, but the judgment also
considers indirect discrimination. There, it appears to establish that
workplace policies of “Stonewall Law” which are implemented
through the Diversity Champions Scheme – e.g. the removal of single
sex spaces – DOES in fact generate legal liability”.

10. Bailey thanked everyone who has supported her and said that she will consider carefully next steps with her legal team.
